St. Johns Home is a not-for-profit multi-level residential health care facility, which has provided a continuum of care for the elderly in Rochester for ninety years. The nursing facility is home to 471 residents, and there are 64 apartments located on an eight-acre campus next to Highland Park. Specialty programs include mental health for wandering residents and comfort care for terminally ill residents. St. Johns Home also offers programs which provide services to people who continue to live in their own homes, including Home Link long term home health care, HandiCall emergency response system, Respite Care, Day Break and Day Break II adult day health services and a rehabilitation program for those who return home after in-patient rehabilitation.

The Nursing Facility

alberghi Bojnice b&bThe nursing facility provides 230 beds for residents who need long term skilled nursing care, as well as those who are admitted into the Rehabilitation, Mental Health, Comfort Care and Respite Care programs.

St. Johns Home also provides 241 beds for long term care for residents who are able to function more independently. These patients wheel or ambulate to meals, but need nursing supervision and assistance with the activities of daily living and with medications. Building design and furnishings are adapted for use by wheelchair residents.

Rooms are individualized with personal belongings, and residents enjoy use of patios, porches, lounges and the chapel. Religious services are available for all denominations. Resident meals are served in dining rooms unless other arrangements are medically necessary. Menus are planned on a five week cycle, with resident input, offering two main dish choices at each meal. Special diet restrictions are accommodated.

St. Johns Home provides full time Physical and Occupational therapy departments, as well as Speech, Hearing, Rehabilitation, Music and Recreation therapists. The Therapeutic Recreation department plans a full range of activities, including arts and crafts, bingo, bowling, singing, films, games, discussion groups and monthly birthday parties. There is an active Residents Council, plus resident choral, gardening and discussion groups.

There are family support groups, counseling, meetings with staff and a Family Council available to offer support and information.

St. Johns has an in-house medical staff. Residents may ask that their own physician be credentialed at St. Johns. A team of professional mental health providers is available for consultation.

Respite
Respite program offers advanced reservations for stays of 1-6 weeks at the home.Comfort Care
Dignify and comfort during the last months of life are the goals of the Comfort Care program. Services are provided by an interdisciplinary team made up of physicians, nurses, home health aides, therapists, social workers, pastoral care counselors, pharmacists, nutritionists, clinical specialists and volunteers. Emotional support and bereavement counseling are services for the family that continue through a family members illness and the months following death.Day break II
Day Break II is a social adult day program held Tuesdays and Thursdays at United Church of Christ Congregational in Webster. Day Break II provides an outlet for older adults, people with disabilities, confused older adults or those without the opportunity to enjoy companionship and social interaction outside of their own homes. Day Break II clients enjoy indoor and outdoor activities including games, baking, crafts, exercise and educational programs.RehabilitationThe Rehabilitation program at St. Johns Home helps restore a persons highest level of functioning and independence after an illness or injury. Professional individual assessment is used to create a complete therapeutic rehabilitation plan that results in discharge to ones home or to a lower level of care.HomelinkThe Homelink Program is a long term home health care program that brings the services of St. Johns Home into a clients home.HandicallHandicall is a two-way voice emergency response system offered by St. Johns Home. Over 650 clients wear the signal button which provides immediate access to Emergency Assistance.
