This facility is a private, skilled nursing home. It is located in a residential neighborhood with a bus stop in front of the home. There are 36 double rooms and 8 single rooms, each with a sink, shower and toilet. Blossom Health Care Center accepts Medicare and Medicaid patients. Visiting hours are from 11:00 a.m. to 9:00 p.m. The daily rate includes staff physician services for required routine visits.

There are a total of 80 residents in the home. All applicants are accepted, with the exception of patients with a primary diagnosis of alcoholism and drug abuse. Short term admissions of not less than 30 days are permitted.

This facility is suitable for wheelchairs in all areas of the building. Protestant and Catholic religious services are regularly conducted. There are lounges and conference rooms on each floor, and patios and porches for the patients use. The main meal is served at noon; snacks are served before bedtime. An alarm system has been installed as a safeguard against wandering residents.
