ALTERNATIVE LIVING ACCOMMODATIONS

When it becomes impossible to live alone, a number of alternative living arrangements are available to help maintain an independent lifestyle. These programs have varying levels of support and services, but they all share the goal of providing ways to live safely and independently outside of an institutional setting like an Adult Care Facility or Nursing Home.

Support can include: meal preparation, communal dining, housekeeping, social activities, assistance with financial management, transportation service and assistance with long term care planning. Some of the programs are subsidized by public funding, and each program has a specific method of payment which you should ask about when you contact them.

Shared Housing

Shared Housing are homes in which residents have their own bedrooms, but share bathrooms and common space such as living and dining rooms and kitchens, as well as household expenses. For information about locations in Brighton, Brockport and Fairport, contact the Housing Council, 546-3700.

Apartments For Seniors

Some housing agencies offer special services to help seniors find appropriate apartments. These apartments generally offer only minimal support in the activities of daily living (ADL). Agencies include:

Apartments Affiliated With Long Term Care Facilities

Some apartments which offer only minimal "ADL" support have affiliations with Adult Care Facilities or Nursing Homes. These affiliations provide apartment residents enhanced access to the long term care facilities if they ever need this type of care.

Adult Care Facilities

Adult Care Facilities are certified by the State Department of Social Services. They offer some supervision as well as coordination of medications and medical services. These facilities provide room, meals, laundry, recreational activities and limited personal care services in activities of daily living. They do not provide nursing care.

Terminal Care

Terminal care is designed to provide medical, social and spiritual support to those who are dying. Most programs expect a participant to have a prognosis of three to six months duration, and also have a comfort care approach. People in this phase of life usually do not require hospitalization, but will require specialized care from these programs.

Payment source varies, and many health insurance policies often cover much of the cost.

Home Hospice

  • GRHC: 325-1880
  • VNS: 787-2233

Family Hospice

  • Most nursing homes will accept terminal care patients
  • Some nursing homes contract with GRHC or VNS for a formal Hospice program (see services chart)
  • Both GRHC and VNS maintain beds at various in-patient facilities (i.e. GRHC Hospice at St. Marys). Arrangements must be made through GRHC or VNS.

Comfort Care Homes

These volunteer run homes are able to provide terminal care to two patient-guests at a time. Services can continue to be provided through GRHC or VNS.
