ERROR MSGLong Term Home Health Care Program
The long term home health care program provides nursing home level care to patients in their home. Referred to as the 'Nursing Home Without Walls', the program coordinates a comprehensive range of health, social and environmental services tailored to the patient's specific needs. Patients are monitored 24 hours a day, 7 days a week. Care may include: skilled nursing care, aide service, Meals-On- Wheels, therapies and other medical monitoring as necessary.
The following organizations have a long term home health care unit. Fees are covered by Medicaid, Medicare, private insurance and private payment.

Home Health Care (Skilled)
The services which are provided are of a preventive, therapeutic, rehabilitative health guidance and/or supportive nature to persons at home. Medical professionals make home visits if the older person is unable to get out for treatment. Physical, occupational, speech therapists, nurses and social workers are part of the home health team. Also included are: medical supplies, equipment and appliances suitable for use in the home.

Advocacy Procedures for Clients Receiving Home Health Care
Clients have rights to voice grievances to the service provider regarding their care and service. If a client is unable to resolve his/her complaint with the provider, the complaint may be filed with:
