Dedicated to the ViaHealth concept of providing a full range ofcoordinated services, the team may also include representatives fromViaHealth affiliates, Independent Living for Seniors and ViaHealthHome Care. They play an active role in the discharge planning stages ofcare delivery for the rehabilitation resident.
