Specialty Services (Sub-acute or Transitional Care)

Transitional Care is a bridge of care between hospitalization and home, oris an alternative to hospitalization. It includes rehabilitativeservices, medical treatments, palliative and comfort care, andconsultative planning with home health care agencies for home services.

Our interdisciplinary team of professional care-givers under thedirection of our Medical Director, works closely with the patient, familyand primary care physician to make the transition from the hospital moremanageable. The Transitional Care Unit provides treatment for and allowsrecovery from a variety of medical and/or surgical conditions, such aspulmonary or cardiac disease, hip fractures, joint replacements and majorsurgery.

Services offered: medical care, nursing care,psychosocial support, discharge planning, physical therapy, occupationaltherapy, speech pathology, recreational therapy, respiratory therapy,nutritional service, intravenous (IV) therapy, spiritual support, ahospice program through Genesee Region Home Care, geriatric consultation,radiology and clinical laboratory services, ophthalmology, dental,audiology, podiatry consults, and the involvement of volunteers.

The following amenities are also offered: a physical therapygymnasium, beautician services, in-house laundry, services, TV, phones,gift shop, and reduced-rate parking.
