Geriatric Evaluation Program

Most older people live healthy and active lives. But, with advancingyears, some individuals encounter chronic illnesses or disabilities. TheGeriatric Evaluation Program was formed at Monroe Community Hospital tohelp address the long-term health concerns faced by older individuals andtheir families. This consultative program was one of the first of its kindin the nation, and has gained national recognition since it inception in1980.

The program is dedicated to helping older people maintain good healthand independence throughout the aging process. A team of health careprofessionals, including a physician, registered nurse, and social worker,thoroughly assess the health and functional ability of each individualpatient by conducting a series of examinations and visits. Thiscomprehensive process allows the team to fully understand the heath careneeds of each person in order to prepare a customized care plan for eachindividual.
