We help you siftthrough the myriad of programs, services and facilities that offer seniorassistance to find one that's right for you and your family.Start by making a telephone call to 1-800-449-4324. That call puts you intouch with experienced professionals at ElderChoices Helpline. They'll goover your current situation, take a look at your options, provide you withthe information you need and refer you to the right people -- free ofcharge.
If you want or need more assistance, ask for ElderChoices Care Management.Someone will visit you and your family at home or the hospital to talkabout your needs and preferences. Then we'll research your alternativesand visit care providers. Once a decision has been made, we'll make thearrangements for you so everything goes smoothly. Afterwards, we'llmonitor care services to make sure they continue to meet your family'sneeds.
