Rehabilitation Services

Hill Haven's 28-bed specialized rehabilitation unit provides intensivedaily therapy with a focus on the resident's return home.

Our interdisciplinary team is tailored to the resident'sindividualized needs and includes professionals from some or all of thefollowing disciplines: medicine, rehabilitation nursing, physiatry,physical therapy, occupational therapy, speech therapy, psychiatry,social work, pastoral care, clinical nutrition and therapeutic recreation.

Dedicated to the ViaHealth concept of providing a full range ofcoordinated services, the team also includes representatives fromViaHealth affiliates, Independent Living for Seniors and the ViaHealthHome Care. They play an active role in the discharge planning stages ofcare delivery for our rehabilitation residents.
