| Rehabilitation ERROR MSGWhen someone you know needs short-term rehabilitation care (after orthopedic surgery, stroke, or a debilitating illness) you can recommend Kirkhaven with complete confidence and peace of mind. That's because at Kirkhaven, we're small enough to render personalized care, but we're fully equipped to offer a variety of specialized services. While we have been providing rehabilitation services almost since inception we established dedicated rehab beds in 1995 and with the help of a generous donor we now have an entire wing dedicated to rehab and transitional care. Transitional care is a broader term and includes rehabilitation (see expanded description under heading Speciality Services). Our goal in rehabilitation care is to help residents become as independent as possible . . . as quickly as possible. And there's no feeling of "assembly line" care here. The Personalized Care Plan ERROR MSG Again and again, our residents tell us they don't have that "large institution" feeling when they're at Kirkhaven. Each resident works with the Kirkhaven rehabilitation team in developing and implementing a Personalized Care Plan with specific, step-by-step goals. The team can include: a geriatrician, nurse, certified nursing assistant, social worker, physical therapist, physical therapy assistant, occupational therapist, certified occupational therapy assistant, speech pathologist, audiologist, recreational therapist and dietitian. Once a week, the team meets to review the progress made and make any adjustments to the plan. We're always flexible in meeting a resident's needs. Medical Supervision is On-Site At Kirkhaven, the care is always "up close and personal." Geriatricians are on-site; they have admitting privileges to The Genesee Hospital, which is right next door. Furthermore, our staff regularly communicates with the resident's primary physician. Fully Trained, Dedicated Staff We are blessed with an exceptional staff of fully trained, dedicated people. To stay current with the latest rehabilitation techniques and equipment, their skills and training are updated on a regular basis. Our nursing staff is trained in the use of PICC lines and central lines for residents who need IV therapy. The Resident's Family is Part of the Team We encourage family members to participate in a resident's rehabilitation. Their presence often aids the recovery process, and so, they're welcome to visit at any time and even take part in the care conference. We provide support to families to help them cope with the lifestyle changes caused by a loved one's need for rehabilitation. Physical Therapy In physical therapy, residents receive training to improve their "navigational" skills: moving around their room, the bathroom, the nursing unit, outdoors and on stairs. Muscle-strengthening and balancing exercises help improve overall functioning. Some residents learn to use special equipment such as a brace, prosthesis, wheelchair, walker, or cane. Training that simulates the home environment is also provided for those planning to return home. Occupational Therapy In occupational therapy, we help residents regain independence in functional skills such as eating, bathing, dressing, and everyday household/kitchen activities. To reach these goals, the resident may perform a variety of upper-body exercises involving range of motion, strengthening, coordination and endurance. Speech/Language Therapy cheap hotels in Chania A number of medical conditions can impair speech and language skills. The speech pathologist works closely with the resident to rebuild those skills. If swallowing is a problem, the dietitian and speech pathologist work together to ensure progress. Recreational Therapy cheap hotel room Esbjerg Many of our rehabilitation residents opt to enjoy leisure-time activities on their own, such as using the videotape library or watching TV. Others choose to take part in our organized program. Based on the resident's needs and interests, the recreational therapist suggests activities that will stimulate the resident. The positive effects gained from music, gardening, singing, and so on, often improve the resident's medical condition. All the Comforts When a resident moves into one of our rehabilitation rooms, all the amenities are there. The rooms were decorated by a designer who specializes in healthcare interiors. Each room has a phone, TV and VCR, as well as built-in utility hook-ups, window and privacy curtains, wardrobe closets and dressers, and side chairs. We Keep in Touch After the Rehabilitation Period Before the discharge date, a home assessment may be done in order to ensure that the person can function safely and effectively at home. The team works with each resident to determine the need for transitional home care and assistance, and the social worker makes appropriate referrals to ensure that all areas of independent living are addressed. After a resident is back home, we make a follow-up call to see how they're doing. We even have Rehabilitation Reunions every two years! "Graduates" are invited back to enjoy a special luncheon. If a rehabilitation resident would function better under long-term care in a skilled-nursing facility, we can help the family make that decision. Of course, where appropriate, we recommend Kirkhaven!
