Speciality Services (Transitional Care or Sub-Acute Care) Knokke-Heist accommodationBoth health care and elder care are seeing substantial changes and one by product has been the emergence of a new level of care that is being called transitional care or sub-acute care. Kirkhaven's role is changing as well. While home care, adult homes and some other nursing homes may specialize in the more well elderly, the population we serve tends to be more acute. The state issues guidelines on how to rate the acuity of residents. Our success in dealing with higher acuity long term care cases and complex rehabilitation situations has positioned us well for providing high quality transitional care. Home care and other services are providing care for many people who traditionally would have been served in the nursing home setting. The pressure to reduce health care costs is another force that is driving the development of transitional care or sub-acute care facilities. Dianamarka HotelesKirkhaven has been providing rehabilitation care in some form for many years in developing its niche in caring for the very frail elderly. We are actively pursuing expansion of this level of service. While rehabilitation is currently a big part of our transitional care services,we are also broadening the number of conditions and level of acuity that we can care for. Kirkhaven is training staff in handling increasing levels of acute conditions in order to meet the demand for a more affordable health care option for those who do not need to be in the hospital. Kirkhaven staff are trained in the use of PICC lines and central lines for residents who need IV therapy. As of January of 1998 we have an entire wing designated specifically for Transitional care residents. This wing has been named The Anderson Transitional Care Center and is located on the west wing of our second floor. Transitional care bedrooms come equipped with extra amenities in order to accommodate a shorter stay. Television with VCR is provided, as well as telephone service that is ready for use even in your first few minutes in the room. Our goal is to make your stay with us as comfortable and carefree as possible so that you can concentrate on the important things, like getting stronger and getting back home again. We welcome your inquiries about transitional care or rehab.
