| Private Office My practice is located in the Medical Building attached to the Wood County Hospital. I have privileges in the hospital, in the operating room and I am one of three dentists in the area who cover the emergency room at the Wood County Hospital. These are a few pictures of my private practice office and staff in Bowling Green, Ohio. As you can see, it is a very modern practice. We moved into our new office in September 1999. Our office consists of six operatories and 2 hygiene areas. We have a lab as well as an up-to-date sterilization area.

| St. Vincent - Mercy Children's Hospital This is the hospital where I am doing all of my operating room cases. I take approximately two patients every Friday morning to the operating room. I also take the Medical College of Ohio general practice residents to the operating room with me at Mercy children's Hospital for teaching purposes. These patients are medically handicapped, or small children ages 1 to 4 with massive dental caries and abscessed teeth.

| Medical College of Ohio Hospital I am a clinical associate professor at the Medical College of Ohio dental clinic, as well as a staff member at this hospital.
