| Specialty Services Fairport Baptist Homes discount hotels in KortrijkFairport Baptist Homes in recent years has been providing care management(also called case management) to residents of DeLand Acres, FairportApartments, and Roselawn, and in the spring of 1997 this service wasexpanded, in response to an invitation by t he Fairport/Perinton SeniorLiving Council, to include seniors throughout the Fairport/Perintoncommunity. Senior Options for Independence (SOFI) is a program of non-medical homesupport services that enables older persons to continue livingindependently at home. SOFI through its Community Connections can arrange forfriendly visits, housecleaning and yard work, home repairs and safetychecks, arrangements for meals, transportation, assistive devices (such asa walker or hearing aid), help with bathing or dressing, financial andlegal counseling, long term care planning, and help with benefit forms. If you are 60 or older and/or if you have one or more impairments thatmake it difficult to carry out everyday tasks, you may be eligible forSOFI's services. (You also must be a resident of the town of Perinton orvillage of Fairport.) Fairport Baptist Homes is a partner in this continuing carenetwork, which integrates primary, acute (hospital), and long term carefor persons with chronic illnesses. Health for Life can manage a client'sdoctor visits, hospitalization, ho me care, day care, and/or planning foradmission to a nursing home or adult care facility. Clients have accessto a health care professional 24 hours a day, seven days a week.
