Enriched Housing

The residents of Enriched Housing are independent and active. They enjoythe privacy of their own apartments, and are assured of help through avariety of coordinated support services that are listed below.

Services:

Housing:

    Private one bedroom apartments
    Emergency call cords
    Handicapped accessible
Resident Services:
    Weekly housekeeping and laundry
    Personal care and assistance with bathing
Nutrition:
    3 daily meals (breakfast, congregate noon meal and a bag supper)
    Special diets available
Case Management:
    Coordinating services with individual needs
    Arranging transportation for appointments
    Scheduling medical appointments
    Managing finances
Recreation:
    In-house social and recreational activities
    Regular community outings
Financial:
    Bill paying and money management services
    Assistance in qualifying for available benefits

Qualifications:

To be eligible for Enriched Housing, individuals must:

    Be 60 years of age or older, or be an adult with a special need Require support services to remain independent Be able to take their own medication without assistance Qualify through a medical-social evaluation

Enriched Housing is available to all qualified individuals regardless ofincome; both private pay and SSI applicants are accepted. Cost is $610 permonth for services; plus rent (no more then 30% of income). ForSSI-eligible individuals, Level II reimbursment is accepted. There isone monthly charge for rent and services.

Assisted Living

Assisted Living is a housing program for frail elderly that provideslimited nursing care along with support and social services. AssistedLiving is licensed by both the New York State Department of SocialServices and the New York State Department of Health . This programoperates in partnership with the Monroe County Community Home HealthAgency.

Services:

All the services provided in the Enriched housing program listed above,plus

    Nursing Services, such as dressing changes, medication administration,speech therapy, physical therapy and occupational therapy.

    Assistance with Activities of Daily Living (ADL's) such as bathing,dressing, feeding, and ambulation.

Qualifications:

Bournemouth alberghi b&bIndividuals must be eligible for nursing home care as determined by anagency evaluation. At the same time, the individual must not requireintensive, critical or comprehensive nursing care.

Fees:
